Accident investigation

A workplace accident can have serious consequences for both the employees involved and your organisation. Besides physical damage, it can lead to extended downtimes, reputational damage and even legal consequences. It is therefore essential to thoroughly investigate every accident. At EFAS, we help you conduct an in-depth accident investigation so that you can take the right measures to prevent recurrence and ensure safety within your organisation.

What is an accident investigation?

An accident investigation is the process of analysing the causes and circumstances of an accident. This investigation goes beyond the immediate cause and also looks at underlying factors, such as work processes, safety measures and workplace behaviour. The aim is to get a complete picture of how the accident could have happened and what can be done in the future to avoid similar situations.

Why is an accident investigation important?

Conducting an accident investigation is crucial to improving workplace safety. Not only does this help you prevent repeat accidents, but it also helps improve the safety culture within your organisation. In addition, a thorough investigation shows that you are fulfilling your duty of care as an employer and actively working on the health and safety of your employees.

When is an accident investigation mandatory?

According to the Occupational Health and Safety Act, you are obliged to carry out an accident investigation in case of serious occupational accidents. These are accidents involving fatalities, permanent injuries or hospitalisation. These accidents must be reported immediately to the Netherlands Labour Inspectorate. It is also advisable to carry out an investigation for every incident, however minor, to make structural improvements.

How does an accident investigation proceed?

An accident investigation consists of several phases, identifying all those involved and factors. At EFAS, we take a thorough and practical approach consisting of the following steps:

1

Incident analysis – We collect all relevant information about the accident, such as witness statements, photos and reports.

2

Cause analysis – We investigate both the immediate and underlying causes of the accident. Here, we look at work processes, safety instructions, working conditions and the behaviour of those involved.

3

Report – We prepare a summary report detailing the causes of the accident. We also provide clear recommendations to prevent recurrence and improve safety procedures.

4

Plan of action – Together with you, we draw up a plan of action to implement the necessary improvement measures. This can range from adjustments in work processes to additional training or changes in safety policies.

5

Aftercare and evaluation – After the measures have been implemented, we stay involved to evaluate their effectiveness and whether further adjustments are needed.
